- You have a smartphone with an active data plan.You have been diagnosed with diabetes.You have been diagnosed with heart failure.Your blood pressure is too high, even with medication.You have had a stroke or heart attack in the last 6 months.You have cancer that is currently growing or causing symptoms.You have been diagnosed with dementia and it has been written down by a doctor.
- Group 1: Modified Sodium Watcher Program + Digital Self-Monitoring
- Group 2: Usual care + Digital Self-Monitoring
- No Placebo-Only Group - All patients enrolled in this study will receive some form of active treatment.
- Screening: It may take up to 3 Weeks to process to see if you qualify in this trial.
- Treatment: The duration you will receive the treatment varies.
- Follow Ups: You may be asked to continue sharing information regarding the trial for 6 Months after you stop receiving the treatment.
