- I can perform most daily activities without help.My physical ability score is 9 or less out of 12.I have been diagnosed with dementia or have had a stroke.I can walk 400 meters in 15 minutes or less without help, but I can use a cane.
- Group 1: Health Coaching (HC)
- Group 2: Health Education Program (ED)
- No Placebo-Only Group - All patients enrolled in this study will receive some form of active treatment.
- Screening: It may take up to 3 Weeks to process to see if you qualify in this trial.
- Treatment: The duration you will receive the treatment varies.
- Follow Ups: You may be asked to continue sharing information regarding the trial for 6 Months after you stop receiving the treatment.
