- I experience moderate to severe pain that affects my daily activities and enjoyment of life.I have had muscle or joint pain in my back, neck, or limbs for over 3 months.You must have at least a little bit of depression, as determined by a PHQ-8 score of 5 or higher.I have an appointment with my primary care doctor in the next 2-4 months.I experience moderate to severe pain that affects my daily life.I have had pain in my back, neck, or limbs for more than 3 months.I do not have severe medical conditions that would stop me from joining.You have thoughts of hurting yourself, or you have severe difficulty hearing, speaking, or thinking clearly.I am open to trying new pain treatments.You cannot have been involved in the researcher's previous study or served on the project's patient panel.I am open to trying new treatments for pain.
- Group 1: Coaching and Decision Aid
- Group 2: Control
- No Placebo-Only Group - All patients enrolled in this study will receive some form of active treatment.
- Screening: It may take up to 3 Weeks to process to see if you qualify in this trial.
- Treatment: The duration you will receive the treatment varies.
- Follow Ups: You may be asked to continue sharing information regarding the trial for 6 Months after you stop receiving the treatment.

Secondary objectives include assessing variations from baseline for Brief Pain Inventory (BPI) Intensity Scale at 3 months and Patient-Reported Outcomes Measurement Information System (PROMIS) Sleep Disturbance Short Form scores over both 3 month and 6 month intervals."
