This trial aims to help patients and their carers reduce use of CNS drugs, reducing falls and injuries.
Who is the study for?
This trial is for older adults with dementia who are patients at a Kaiser Permanente Washington clinic and have been on CNS-active medications for over three months. It aims to help them, along with their caregivers and doctors, reduce medication use that could lead to falls.Check my eligibility

The STOP Falls D Educational Intervention is being tested. This program educates dementia patients, their care partners, and primary care providers about the risks of certain medications and how to work together to prevent falls.See study design

Eligibility Criteria:
This trial includes the following eligibility criteria:- I am an older adult getting primary care at a Kaiser Permanente clinic.I have been taking medication for my brain or nerves for 3 months or more.I am a caregiver for an older adult with dementia.I am an older adult with a current cancer diagnosis.I live in a skilled nursing facility.I am an older adult receiving hospice or palliative care.I have been diagnosed with dementia or take medication for it.My care partner is 18 years old or older.
