Study Summary
This trial is testing whether routine assessment of patient-reported health status using the Kansas City Cardiomyopathy Questionnaire-12 (KCCQ-12) impacts clinical processes of care among adult outpatients with heart failure. The primary objective is to evaluate clinician inertia by measuring the clinician action rating (CAR). Secondary outcomes include individual components of the composite outcome, therapy rates, resource utilization, and patient experience.
