Study Summary
This trial aims to reduce hospital readmissions by providing a post-discharge income supplement to vulnerable older adults in an enhanced care plan. It seeks to combine healthcare and social needs to improve healthcare delivery.
Who is the study for?
This trial is for older adults aged 55 or over who have been recently discharged from the hospital and are part of an enhanced care program. They must understand the consent form, be attending a clinic appointment within 10 days post-discharge, and not have a legal guardian controlling their finances.Check my eligibility
What is being tested?
The study tests if giving a one-time income supplement after leaving the hospital can reduce unplanned readmissions within 30 days for vulnerable seniors with complex medical needs, alongside improved care coordination.See study design
What are the potential side effects?
Since this trial involves income supplements and care planning rather than medications, there aren't typical drug side effects. However, participants may experience changes in stress levels or health due to financial relief.
