Eligibility Criteria:
This trial includes the following eligibility criteria:- I am a nursing home resident and not a candidate for hospital or acute care.I cannot be admitted to the hospital for care because I have COVID-19.My health condition is stable.I am on dialysis and not fit for certain hospital care.I have been discharged and am not currently eligible for hospital or acute care.I am able to make my own health decisions or with help from a health care proxy.My main health issue matches one of the specific conditions the study is targeting.I can make my own medical decisions or have someone who can decide for me.I need care that is more intensive than what a regular hospital or acute care hospital can provide.I am 18 years old or older.
