The purpose of this study is to see if it is safe and effective to give CPI-1189 to patients with AIDS dementia.

Advanced HIV infection can cause AIDS dementia (brain damage due to HIV leading to loss of memory and muscle control). CPI-1189 may be able to postpone AIDS dementia or slow it down.

Antiretroviral Therapy for HIV

This study is evaluating whether it is safe and effective to give indinavir plus ritonavir plus 2 NRTIs to HIV-infected patients who need early intervention treatment.

My HIV treatment initially worked, lowering my viral load below 400 in two tests.

My HIV treatment failed twice with viral loads between 400 and 20,000.

I have not used at least one of the specific HIV medications before.

I have been on HIV treatment including indinavir or nelfinavir for at least 16 weeks.

Pivotal Trial

The primary purpose of the study is to determine the efficacy of an investigational therapy called DCVax(R)-L in patients with newly diagnosed GBM for whom surgery is indicated. Patients must enter screening at a participating site prior to surgical resection of the tumor. Patients will receive the standard of care, including radiation and Temodar therapy and two out of three will additionally receive DCVax-L, with the remaining one third receiving a placebo. All patients will have the option to receive DCVax-L in a crossover arm upon documented disease progression. (note: DCVax-L when used for patients with brain cancer is sometimes also referred to as DCVax-Brain)

I stopped taking steroids at least 10 days before my leukapheresis.

My initial treatment was surgery to remove as much of the tumor as possible, followed by radiation and Temodar chemotherapy.

I agree not to receive certain cancer treatments except for temozolomide as per the Stupp Protocol while on DCVax-L.

My tumor sample from surgery is enough for the protein test.

I have enough DCVax-L available for treatment after it was made.

I have been diagnosed with Grade IV brain cancer in one side of my brain.

My bone marrow, liver, and kidneys are working well.

I am between 18 and 70 years old and can legally consent.

RATIONALE: Estrogen can stimulate the growth of breast cancer cells. Hormone therapy using triptorelin, exemestane, and tamoxifen may fight breast cancer by blocking the use of estrogen. It is not yet known whether giving triptorelin together with exemestane is more effective than triptorelin and tamoxifen in treating hormone-responsive breast cancer.

PURPOSE: This randomized phase III trial is studying triptorelin and exemestane to see how well they work compared to triptorelin and tamoxifen in treating premenopausal women with hormone-responsive breast cancer.

This study is evaluating whether a combination of hormones is more effective than a single hormone in treating breast cancer.

I don't have any chronic diseases that would interfere with long-term treatment.

I haven't had any cancer except for skin cancer in the last 5 years.

My breast cancer cannot be removed by surgery and has spread to nearby areas.

I have not had chemotherapy before or after surgery.

I haven't had any cancer except certain skin cancers or non-spreading breast cancer in the last 5 years.

My estrogen levels are considered premenopausal after surgery.

I have never had a blood clot or if I did, my doctor says it's okay.

My breast cancer diagnosis was confirmed through tissue examination.

My cancer has not spread to distant parts of my body.

I have had breast cancer in either the same or the opposite breast before.

I do not have liver disease that would interfere with long-term monitoring.

I do not have kidney disease that would interfere with long-term monitoring.

I am not taking any estrogen, progesterone, or androgen treatments.

I am not taking any other aromatase inhibitors.

I don't have any lung diseases that would interfere with long-term monitoring.

I am fertile and not using birth control methods that don't involve hormones.

I haven't had hormone therapy for breast cancer since diagnosis.

I have never had radiation therapy for ovarian cancer.

I am not currently taking bisphosphonates, or I meet the specific criteria for their use.

I haven't used tamoxifen, SERMs, or hormone therapy for over a year before my breast cancer diagnosis.

I am not currently on any oral or patch-based hormone treatments.

I have not had both of my ovaries removed.

I do not have any detectable cancer in the lymph nodes under my arm.

I am a woman who has not gone through menopause.

I have had surgery to remove lymph nodes under my arm or a biopsy that was negative.

I am not using any hormonal contraceptives.

My cancer is only in my breast and nearby lymph nodes.

My cancer was entirely removed by surgery.

I don't have heart disease that would interfere with long-term monitoring.

I have had surgery or radiation on the lymph nodes under my arm due to cancer.

My cancer is positive for estrogen or progesterone.

I have had a condition where my lymphocytes grow abnormally.

RATIONALE: Estrogen can cause the growth of breast cancer cells. Hormone therapy using letrozole may fight breast cancer by lowering the amount of estrogen the body makes. It is not yet known whether letrozole is more effective than a placebo in treating patients with hormone receptor-positive breast cancer.

PURPOSE: This randomized phase III trial is studying letrozole to see how well it works compared with a placebo in treating postmenopausal women who have received hormone therapy for hormone receptor-positive breast cancer.

This study is evaluating whether a drug called letrozole can help treat breast cancer.

You cannot participate in this study if you have any of the following conditions:

You have had a bone fracture in your wrist, hip, or spine that was not caused by an injury.

If you have high cholesterol, you must have it under control with medication or lifestyle changes for at least one year. If you have a history of diabetes, high blood pressure, obesity, smoking, high triglycerides, heart disease, or a family history of early-onset heart disease, you may not be eligible to participate.

You have had another type of cancer in the past, but you are considered to be cured for at least 5 years, and your doctor believes the cancer is unlikely to come back. However, if you had certain types of non-invasive cancers in the past 5 years, you may still be eligible.

RATIONALE: Drugs used in chemotherapy, such as AP5346 and oxaliplatin, work in different ways to stop the growth of tumor cells, either by killing the cells or by stopping them from dividing.

My cancer has spread and cannot be surgically removed, with no cure available.

My cancer has returned and this has been confirmed by tests.

I do not have major neurological or psychiatric issues like dementia or seizures.

My cancer is a type of squamous cell carcinoma located in my head or neck.

My health limits me from doing any physical work.

I can take care of myself and perform light activities.

I do not have severe numbness, tingling, or pain in my hands or feet.

My kidney function is within normal limits.

My tumor can be biopsied with a simple procedure.

I do not have unstable chest pain or irregular heartbeats.

Oxaliplatin

RATIONALE: Gefitinib may stop the growth of tumor cells by blocking some of the enzymes needed for cell growth. PEG-interferon alfa-2a may interfere with the growth of tumor cells and slow the growth of skin cancer. Giving gefitinib together with PEG-interferon alfa-2a may kill more tumor cells.

PURPOSE: This phase I/II trial is studying the side effects and best dose of PEG-interferon alfa-2a when given together with gefitinib and to see how well they work in treating patients with unresectable or metastatic skin cancer.

The purpose of this study is to determine whether a one-time intracoronary infusion of Ad5FGF-4 is effective in reducing the time to onset myocardial ischemia as measured by exercise treadmill testing and improving myocardial blood flow as measured by SPECT imaging. Exercise capacity, angina functional class, patient symptoms and quality of life will also be evaluated to characterize the efficacy of Ad5FGF-4. Short-term and long-term safety of Ad5FGF-4 will also be evaluated.
