How Sodium Nitroprusside Affects Patients

Nitroprusside is a drug that relaxes the walls of blood vessels, especially veins, causing them to expand and increasing blood flow. It does not have an effect on other muscle groups, such as the uterus or duodenum.

How Sodium Nitroprusside works in the body

Sodium nitroprusside works in two ways. First, it breaks down into molecules that bind to hemoglobin, forming a compound called cyanmethemoglobin. This compound binds to cyanide ions, which are toxic, and removes them from the body. Second, it breaks down further into nitric oxide, which increases production of a molecule called cGMP. This molecule helps reduce calcium in the cells, leading to relaxation of the blood vessels and a widening of the arteries.

The amount of Sodium Nitroprusside is contingent upon the determined condition, including induction of Intraoperative Hypotension, Hypertensive crisis and Intraoperative Blood Loss. The measure of dosage is subject to the method of delivery noted in the table beneath.

The goal of this clinical trial is to learn if increasing the dose of diuretics to achieve a higher urine sodium target produces better clinical results when treating patients hospitalized with acute heart failure when compared to lower urine sodium target and standard of care. The main questions it aims to answer are: 1. Does targeting a higher urine sodium goal achieve greater natriuresis and diuresis? 2. Does targeting a higher urine sodium goal reduce frequency of hospital readmissions? 3. Does targeting a higher urine sodium goal reduce hospital length of stay? Researchers will compare natriuresis-guided arms with standard of care to see if targeting higher natriuresis goals improves significantly over current practice. Participants will submit urine samples at routine intervals after being given diuretics to evaluate urine sodium concentration. If urine sodium is low then diuretic dose will be increased.

Empagliflozin for Acute Heart Failure

The objective is to study in a prospective, interventional, single arm, cohort study the potential synergistic diuretic effect of empagliflozin, in addition to furosemide, in hypervolemic patients admitted with acutely decompensated heart failure and diuretic resistance at the McGill University Health Centre (MUHC). The investigators hypothesize that the sodium-glucose cotransporter-2 (SGLT-2) inhibitor empagliflozin will enhance the diuretic effect of furosemide in patients with acutely decompensated heart failure, moderate to advanced chronic kidney disease, and underlying diuretic resistance, as identified by the three-hour urine output post diuretic administration on the first day of the study, compared with furosemide alone.
