Latanoprostene Bunod is a medication used to treat open-angle glaucoma and ocular hypertension. It was approved by the FDA in 2017 and is the first prostaglandin analog with one of its metabolites being nitric oxide, giving it an extra effect of relaxing tissues and cells. Latanoprostene Bunod is similar to latanoprost, but it has an NO-donating moiety instead of the isopropyl ester found in latanoprost.

How Latanoprost Affects Patients

Upon taking an appropriate dose of latanoprost bunod, the drug will start to lower the pressure in the eye. The maximum effect will be seen after 11 to 13 hours.

How Latanoprost works in the body

Open-angle Glaucoma (OAG) is a condition that causes vision loss. It is often caused by high eye pressure, called Occular Hypertension (OHT). To help treat OAG, medications are used to reduce and maintain the pressure. Latanoprostene bunod works by two different pathways. It increases the flow of aqueous humor out of the eye by making the pathways wider, and it also reduces resistance to the flow by changing how cells contract. Both of these effects result in lower eye pressure and help prevent vision loss.

Long-term use of latanoprost bunod may cause permanent darkening of the iris and increased length, thickness and number of eyelashes, which may be reversible upon discontinuing the medication. While this drug is effective in lowering eye pressure, the side effects are unfortunately quite extreme. After just five months of use, I experienced severe dizziness, vomiting, and flu-like symptoms - to name a few. I stopped taking it immediately and all symptoms subsided. If you're considering using this medication, be very cautious and weigh the pros and cons carefully.

Latanoprost for Increased Pressure in the Eye

I use this medication every evening and it has lowered the pressure in one eye but not the other (which remains high). I have experienced a number of side effects, including but not limited to: fleeting pain in eyes/head, cough, runny nose, blurred vision at times, sensitivity to light, itchy eye lids, increased lash length (must trim outer lashes), back and muscle aches. However, I am still able to see so will continue using this trade-off!

I'm only three nights in and I already feel very dizzy when I wake up. It takes a few minutes to adjust before I can stand, which is really inconvenient. I'm planning on asking my pharmacist and doctor about this medication and if it's the right fit for me.

AI-Based Screening for Glaucoma

The goal of this clinical trial is to learn if implementation of an eye screening program at Federally Qualified Health Center (FQHC) clinics provides results that participants may have glaucoma, and/or other eye conditions (diabetic retinopathy, cataract, visual acuity impairment). The glaucoma screening will incorporate use of an artificial intelligence (AI)-assisted screening tool. This project is called AI-RONA. The main questions it aims to answer are: * How does this eye screening program compare to the rate of glaucoma and other eye conditions detected at other FQHC clinics where the screening program has not been implemented? * Do particpants who screen positive for these conditions adhere to the physician's recommendation for a follow-up examination by an optometrist or ophthalmologist? * Are referral rates for a follow-up comprehensive eye exam by an optometrist or ophthalmologist similar to those implemented by an ophthalmologist using telemedicine (that is, using the results of the screening to make a diagnosis remotely)? * What is the cost-effectiveness of the AI-assisted screening program in diagnosing glaucoma as compared to a physician-guided program? * Are participants completing the screening satisfied with it? * Are physicians at the FQHC clinics administering the screening satisfied with it? Participants will: * Undergo an ocular screening whose goal is to detect glaucoma, diabetic retinopathy, cataract, and/or impairment in visual acuity. If the screening indicates that participants may have these conditions, participants will be referred for a comprehensive eye examination by an optometrist or ophthalmologist. * Following the screening, participants and physicians will complete a survey on their satisfaction with the program.
