After being applied to the eyes, alcaftadine ophthalmic solution, 0.25%, reaches its highest level in the blood (60 pg/mL) in 15 minutes. The level of alcaftadine in the blood drops below 10 pg/mL after 3 hours. Its active carboxylic acid metabolite has a peak level in the blood of 3 ng/mL after 1 hour and drops below 100 pg/mL after 12 hours.

Alcaftadine works by blocking the effects of histamine, which is a chemical released by certain cells in the body. It also stops white blood cells from moving to the area and stops them from becoming activated.
