Treatment Summary

Tadalafil is a medicine that helps to treat erectile dysfunction (ED), pulmonary arterial hypertension (PAH), and enlargement of the prostate. It was approved by the FDA in 2003 for ED and in 2009 for PAH. It works differently than other PDE5 inhibitors like sildenafil, as it has a longer lasting effect and is more selective for PDE5, making it a better treatment option for long-term daily use in PAH.

Effectiveness

How Adcirca Affects Patients

Tadalafil helps treat ED by increasing blood flow to the penis, allowing an erection to occur. It also decreases blood pressure in the lungs in cases of pulmonary arterial hypertension. In cases of BPH, tadalafil may reduce the size of the prostate and relieve its symptoms. This drug has less of a chance of causing visual side effects than other similar drugs.

How Adcirca works in the body

Tadalafil is a medicine that helps relax the muscles in the penis, lungs, and prostate. In the penis, this allows more blood to enter, resulting in an erection. In the lungs, it can reduce high blood pressure and improve oxygen flow. In the prostate, it helps reduce inflammation and tissue growth. Tadalafil also has less of a chance to cause side effects related to vision than other similar medicines because it binds to a different enzyme in the eye.

Adcirca for Pulmonary Arterial Hypertension

My daughter, who has been living with pulmonary hypertension since she was six, just switched from Revatio to Adcirca this month. She says she's already feeling better; less winded and able to exercise more. Let's hope the good results keep coming in!

Adcirca for Inability to have an Erection

The price tag on this one is pretty hefty, but fortunately my insurance company was willing to work with me. As for the side effects, heartburn was the big one for me. It didn't matter what I ate or drank, it would always give me indigestion. The doctor advised that I take Zantac 150mg twice a day to combat it.

Adcirca for Increased Pressure of Pulmonary Circulation

Cialis is great stuff. I was able to achieve and maintain erections for hours on end. My only issue was that my insurance companies didn't want to cover it because my doctor wouldn't lie and say it was for heart problems or blood pressure issues.

Adcirca for Pulmonary Arterial Hypertension

I didn't experience any benefits from this treatment. In fact, it seems to have messed with my hormones and brought back my periods after 15 years of being off them. Now that I've stopped taking the medication, my periods have stopped again.

Exparel/Bupivacaine Mixture for Postoperative Pain in Erectile Dysfunction Surgery

The inflatable penile prosthesis (IPP) is the gold standard for surgical management of erectile dysfunction (ED) and there is no consensus on the best postoperative pain management regimen. In the wake of the opioid epidemic, postoperative pain management is heavily scrutinized. The National Institute of Health estimated over 81,000 individuals died following overdose of any opioids in 2022 alone; of these, over 14,000 deaths were linked to prescription opioids. Thus, strategies to minimize postoperative pain should not only improve patient experience but also lessen the need to escalate to opioid usage.

SAFE Technique for Erectile Dysfunction

18+
Male
New York, NY

Robotic-assisted radical prostatectomy (RALP) has become the standard of care in the management of localized prostate cancer. However, post-prostatectomy erectile dysfunction (ED) and urinary incontinence still pose a challenge that adversely affects the patient's quality of life. Hydrodissection (HD) was introduced in 1987 in the medical field and it was tested for the first time during a retropubic radical prostatectomy back in 2005. Since then, research has indicated an improvement in erectile function results, presumably because of a less traumatic neural dissection and a decreased risk of neuropraxia. In an effort to achieve a balance between oncological excision and functional preservation, prostate surgery is evolving to incorporate intraoperative real-time evaluation of extracapsular extension. Recently, a micro-ultrasound operating at 29 MHz has been introduced. It provides a resolution down to 70 μ to assess the glandular pattern and cellular density of the prostatic tissue. Given the experience in more than a thousand cases with this technology, and the growing literature showing promising results in the detection and staging of prostate cancer; the researchers decided to implement this tool as guidance for hydrodissection of the layers of the lateral prostatic fascia. Robotic-Assisted Laparoscopic Prostatectomy and SAFE (Saline assisted fascial engorgement) nerve preservation guided by Microultrasound (MUS) is a prospective randomized controlled trial designed to evaluate the impact of normal saline solution hydrodissection (HD) on erectile function outcomes after RALP. This innovative approach consists of a transrectal ultrasound-guided HD using a high-resolution Microultrasound (MUS) (ExactVu micro-ultrasound, Exact Imaging, Markham, Canada). The technique will be applied to those patients who undergo grade 1, 2, or 3 NS approach taking into account the grading system proposed by Tewari et al. The hypothesis is that the SAFE technique could minimize the risk of a traumatic neurovascular bundle dissection while assessing the prostatic capsule integrity with a high-resolution MUS. The researchers also hypothesize that the use of SAFE along with RALP will optimize the post-surgery recovery of erectile function.

Optilume vs Rezum for Enlarged Prostate

18+
Male
Montréal, Canada

Lower urinary symptoms (LUTS) affect older men and their frequency and severity increase with age. According to Berry et al., the histological diagnosis of benign prostatic hyperplasia (BPH) increases from 25% in the group age between 40 to 49 years to 80% in the group age between 70 to 79 years (1). However, this hyperplasia can lead to a bladder outlet obstruction (BOO) secondary to benign prostatic enlargement (BPE), causing storage and voiding symptoms, associated with a considerable impairment of the quality of life (2). Surgical therapy of BPH has continuously evolved in recent years. The current gold standard for the endoscopic treatment of BPH is the transurethral resection of the prostate (TURP). Other innovative techniques were developed including Urolift, transurethral laser enucleation (holmium, thulium) and Aquablation therapy. One of the latest technologies for the management of BPH are Optilume BPH Catheter System (Optilume®, Urotronic Inc, Minneapolis, USA) and Rezum Water Vapor Therapy (Rezum System, Bostocn Scientific, Marlborough, MA). The purpose of the present study is to compare between patient experience, symptom relief and functional improvement when using the Optilume BPH Catheter System (Optilume®, Urotronic Inc, Minneapolis, USA) and Rezum Water Vapor Therapy (Rezum System, Boston Scientific, Marlborough, MA).
