It was approved by the FDA in 1991 and is available as a pill, a dissolving tablet, and an injection. Recently, a new oral film form of ondansetron has been developed to make it easier for people experiencing nausea to take the medication.

How Ondansetron Hydrochloride Affects PatientsOndansetron blocks the serotonin 5-HT<sub>3</sub> receptors in the body that are responsible for causing nausea and vomiting. Chemotherapy drugs can release serotonin from the intestines that trigger these symptoms, and ondansetron helps to counteract this. Studies have shown that taking ondansetron can lead to a slight increase in the time it takes for electrical signals to travel through the heart, but this effect has not been seen in those taking it orally. Additionally, it has been found to have no effect on esophageal and gastric motility, lower esophageal sph
How Ondansetron Hydrochloride works in the bodyOndansetron works by blocking a reflex in the body caused by serotonin. Serotonin is a chemical released from cells in the small intestine, which can cause nausea and vomiting. Ondansetron blocks this reflex by blocking the serotonin receptors, located on neurons in the peripheral and central nervous systems. While it seems to work for post-operative and chemotherapy-induced nausea and vomiting, it is not yet known how it affects opiate-induced emesis.

Ondansetron Hydrochloride Toxicity & Overdose RiskThere have been reports of some unusual side effects after taking large doses of ondansetron, such as temporary blindness, low blood pressure, and seizures. However, these reactions went away completely after treatment. Ondansetron has not been tested in pregnant women, so it is not recommended for use during pregnancy. Additionally, nursing mothers should not take ondansetron as it is not known if it is excreted in breast milk. There is also not enough information to provide dosage recommendations for children 3 years of age or younger.

Aprepitant for Postoperative Nausea and Vomiting

Postoperative nausea and vomiting (PONV) are a frequent and debilitating complications after surgery, affecting up to 80% of patients at high risk in the absence of prophylaxis. Despite the rigorous application of the recommendations from the American Society of Anesthesiologists (ASA) at CHUM, a recent local study reveals a prevalence of 25% PONV at home after outpatient surgery. However, the therapeutic options at home remain limited. This study aims to evaluate if the addition of 40 mg aprepitant to a multimodal strategy for preventing PONV improves clinical outcomes in high-risk patients undergoing outpatient surgery.

Autonomic Neural Blockade for Postoperative Symptoms in Bariatric Surgery

The purpose of this research is to evaluate if autonomic nerve block (ANB- blocking pain and nausea signals) decreases pain and anti-nausea medication requirements as well as the experience of pain/nausea during the first 72 hours after sleeve gastrectomy or gastric bypass surgery. Participants will be randomly assigned either to the standard of care or the ANB group before surgery. As part of standard of care, in both groups, laparoscopic bariatric surgery will be initiated with local anesthetic injected into the abdominal wall. In the ANB group, participants will be given an additional injection of local anesthetic medication to block nerves on and around the stomach.

Dexmedetomidine + Dexamethasone for Postoperative Pain

The goal of this clinical study is to improve upon the standard practice of utilizing nerve blocks to enhance the satisfaction of patients undergoing shoulder, or foot and ankle surgery regarding their quality of pain control, the duration of their nerve blocks, as well as the avoidance of opioid medications and subsequently their undesirable side effects. The researchers hypothesize that dexmedetomidine and dexamethasone (Dex-Dex) will work synergistically as adjuncts in a low, non-weight based formula. This would allow the investigators to improve patient satisfaction by providing them with a superior means of pain control that is longer in duration with a decreased probability of unwanted side effects. The proposed clinical study will be a prospective, randomized control trial.

ML-Based Intervention for Vomiting in Pediatric Cancer

The goal of this single arm trial is to learn if a machine learning (ML) model predicting the risk of vomiting within the next 96 hours will impact vomiting outcomes in inpatient cancer pediatric patients. The main questions it aims to answer are whether an ML model predicting the risk of vomiting within the next 96 hours will: Primary 1\. Reduce the proportion with any vomiting within the 96-hour window Secondary 1. Reduce the number of vomiting episodes 2. Increase the proportion receiving care pathway-consistent care 3. Impact on number of administrations and costs of antiemetic medications Newly admitted participants will have a ML model predict the risk of vomiting within the next 96 hours according to their medical admission information. The prediction will be made at 8:30 AM following admission. Pharmacists will be charged with bringing information about patients' vomiting risk to the attention of the medical team and implementing interventions.
