Effectiveness

How Megace Affects PatientsMegestrol mimics the effects of the hormone progesterone, such as changes in the lining of the uterus, a higher body temperature, and causing a period-like bleed when combined with estrogen. It has some effects similar to glucocorticoid and mineralocorticoid, but does not have any estrogenic, androgenic, or anabolic activity. The exact way it helps with anorexia and cancer is unknown. Megestrol was first used as a contraceptive in 1967, and later used to treat cancer.
How Megace works in the bodyWe don't know exactly how megestrol acetate works to help people with anorexia and cachexia, but we think it might have something to do with suppressing the hormone luteinizing hormone. We also think that its effects on weight gain could be related to its ability to increase appetite or affect metabolism. Megestrol may also be able to interfere with the production of hormones that stop fat cells from producing fat.

Decision Support Tool for HIV Treatment

This study is testing software designed to help healthcare providers choose the best HIV treatment combinations for their patients. HIV medicines, known as antiretroviral therapy (ART), can be complex to manage because the right regimen depends on many factors-such as drug resistance, other health conditions, and medication schedules. Many people with HIV are cared for by general clinicians who may not have access to HIV specialists, which can make treatment decisions more challenging. In this study, healthcare providers will use patient cases to compare standard HIV treatment resources with a new clinical decision support tool that gives evidence-based ART recommendations at the point of care. The investigators hypothesize that using the tool will help providers select treatment plans that better match clinical guidelines, make decisions faster, reduce mental effort, and increase overall satisfaction with the prescribing process.

Palliative Care Interventions for Serious Illness

This stepped-wedge, cluster randomized pragmatic trial among 9 MedStar hospitals for patients with serious illness and unmet palliative care (PC) needs will test two interventions embedded within the electronic health record (EHR): (1) a PC needs triggered alert to opt-in to PC consults nudging hospital clinicians to order specialty PC consults for eligible inpatients, and (2) a palliative care needs triggered alert with an opt-out to palliative care consults. The trial will compare the interventions effects to usual care, focusing on completed PC consults during the hospital encounter and other secondary outcomes. The trial also includes an embedded mixed methods study to explore factors influencing the effectiveness and equity of intervention implementation.

WiseApp for HIV

This study aims to conduct a 12-month randomized controlled trial to adapt the mobile app, WiseApp, and a smart pill dispenser for Spanish-speaking people living with HIV (PLWH) in the New York City (NYC) area and La Romana, Dominican Republic (DR). The study will assess the efficacy and sustainability of WiseApp as well as identify barriers with its widespread use among Spanish speakers. With disproportionately high rates of HIV in the New York City area and the Dominican Republic, this project seeks to identify distinct contextual factors related to Spanish speaking people living with HIV and increase the likelihood of engagement with technology and improvements in clinical outcomes.
