Are You a Good Fit for This Trial?
Adults over 18 with a recent diagnosis (within the past year) of specific cancers including colorectal, prostate, lung, breast, gynecologic, hematologic or skin cancer. Participants must speak English and be able to consent. Pregnant women and other vulnerable populations are excluded.Inclusion Criteria

Screening
Participants are screened for eligibility to participate in the trial
Phase 1: Implementation Strategy
Participants complete the ASCENT Questionnaire, are assessed using the Veggie Meter, and participate in semi-structured interviews. Providers participate in interviews.
Phase 2: Intervention
Participants engage in ASCENT patient navigator screenings and consultations, complete the ASCENT Questionnaire, and utilize MyCarePulse.
Follow-up
Participants are monitored for changes in food security, diet quality, and self-efficacy for dietary behaviors.
