This trial tests a nurse-led intervention to help improve quality of life for people living with Alzheimer's and their caregivers, and to make them safer in the community.
Who is the study for?
This trial is for adults over 18 with Alzheimer's or suspected Alzheimer's (FAST score < 4), who can read and speak English. It includes non-paid caregivers providing care to someone with Alzheimer's. Those unable to consent or unwilling to participate are excluded.Check my eligibility

The study tests a nurse-led SUPPORT-D program, designed to improve advanced care planning and quality of life for people living with Mild Cognitive Impairment/Alzheimer’s and their caregivers.See study design

This trial includes the following eligibility criteria:- I am 18 years old or older.I have been diagnosed with Alzheimer's or it is suspected (my functioning is still fairly high).I can read and speak English.You do not have any problems with memory or thinking.I have been diagnosed with Alzheimer's Disease.I am over 18 years old.People with Alzheimer's disease and their caregivers.You take care of someone who has Alzheimer's disease or may have Alzheimer's disease (according to what they have told themselves).
