How Clindesse Affects PatientsClindamycin is an antibiotic that stops the growth of bacteria. It needs to be taken every 6 hours to work properly. Clindamycin can cause severe diarrhea, especially in those who use it for a long time. It can also cause overgrowth of a dangerous bacteria, _Clostridium difficile_. How Clindesse works in the bodyClindamycin prevents bacteria from reproducing by stopping their proteins from being made. It does this by blocking the bacterial ribosome's ability to assemble and translate the proteins. Clindamycin's shape is similar to certain molecules in the bacteria, which helps it to interfere with their protein production. We don't know exactly how it helps to clear up acne, but it may have something to do with its ability to fight the bacteria that causes it.

Clindesse Toxicity & Overdose RiskThe most toxic dose of clindamycin in mice was 2540mg/kg and in rats it was 2190mg/kg. There have not been any reports of overdose yet, but symptoms could include abdominal pain, nausea, vomiting, and diarrhea. In clinical trials, one 3-year-old child was given a daily dose of 100mg/kg for 5 days and only had mild stomach pain and diarrhea. Activated charcoal can be used to help remove unabsorbed drug, but dialysis is not effective. General supportive care is recommended in cases of clindamycin overdose.

Worked very quickly and overnight as advertised. The horrible smell was gone by the morning, which is a huge relief. I usually get bacterial vaginosis (BV) constantly, and my doctor usually prescribes me Flagyl; however, that always makes me violently ill. With this product, I only had mild pelvic pain and white, clumpy discharge; but it's way better than explosive and painful diarrhea.

HIV medicines, known as antiretroviral therapy (ART), can be complex to manage because the right regimen depends on many factors-such as drug resistance, other health conditions, and medication schedules. Many people with HIV are cared for by general clinicians who may not have access to HIV specialists, which can make treatment decisions more challenging. In this study, healthcare providers will use patient cases to compare standard HIV treatment resources with a new clinical decision support tool that gives evidence-based ART recommendations at the point of care. myPACE+ Algorithm for Hypertrophic Cardiomyopathy

This will be a two-arm investigator-initiated randomized controlled study of patients with nHCM and LV ejection fraction ≥50% and NYHA II-III symptoms, normal intrinsic conduction system and pre-existing suitable dual-chamber implantable cardioverter defibrillators (ICD) systems. Patients will be randomly assigned to either personalized accelerated pacing (using the myPACE+ algorithm with mono-fractional exponent) or usual care groups. At baseline and after 3 months of pacing all patients will undergo a CPET, echocardiogram, blood work for NT-proBNP levels and complete the KCCQ-OSS and HCMSQ, questionnaires. The investigator team hypothesizes that personalized accelerated pacing will be safe and improve symptoms and heart-failure related quality of life, physical activity, pVO2, biomarkers (i.e. NT-proBNP), diastolic parameters and cardiac structure.

Amoxicillin Challenge for Penicillin Allergy

The goal of this clinical trial is to learn about dosing when testing to see if a penicillin allergy label can be removed from adults that had been labeled as "penicillin-allergic" previously. The main question it aims to answer is: \- In penicillin-allergic patients that are at low risk of having an allergic reaction, is a one-dose oral challenge with amoxicillin (a penicillin-based antibiotic) as safe and effective as a two-dose oral challenge? Participants will, after being identified as having a low-risk penicillin allergy, be administered oral amoxicillin in a controlled setting and then monitored for an allergic reaction. Researchers will compare participants that took one dose of amoxicillin to participants that took two doses of amoxicillin (a small dose and then a larger dose) to see if either group was more likely to develop an allergic reaction.

Isotretinoin vs Tetracycline for Acne

In the effort to find better treatments for Moderate Acne, which often relies on long-term antibiotic use, researchers are exploring alternative options. While Isotretinoin, a Vitamin A derivative, is highly effective for severe acne, its side effects limit its use for milder cases. A recent study from our institution investigated a new approach: weekly Isotretinoin dosing. The results were promising, with acne improvement and no major side effects. This suggests that weekly Isotretinoin could be a successful alternative for moderate acne in both males and females. To validate these findings, investigators propose a randomized controlled trial comparing weekly Isotretinoin to daily Doxycycline over four months. This study could confirm the safety and effectiveness of weekly Isotretinoin, as well as shed light on patient satisfaction, and long-term results compared to standard antibiotics. This research may offer a breakthrough in treating moderate acne while addressing concerns about antibiotic overuse.

Clascoterone Cream for Acne

Mechanism-based acne treatment for transgender patients receiving testosterone currently does not exist and is an unmet medical need. This study explores clascoterone to treat testosterone induced acne. Many treatments we use to treat acne in females cannot be used in transgender males because they interfere with hormone therapy. Androgens have been associated with the development of acne vulgaris. Recently, a topical androgen receptor inhibitor cream (clascoterone) has been FDA-approved for the treatment of acne. However, clinical trials of clascoterone have excluded participants on exogenous hormones. Clascoterone has been hypothesized to be effective in the treatment of acne in transgender male participants on masculinizing hormone therapy, but it has never been studied or reported in the literature.

Exercise Program for Hypertrophic Cardiomyopathy

The EXCITE-HCM study is a randomized, controlled, blinded clinical trial designed to evaluate the effect of moderate intensity exercise training versus usual physicial activity on the improvement of HCM-related symptoms and cardiac function. About 70 participants will be recruited and randomized on a 1:1 ratio to either moderate intensity training or usual physicial activity interventions. Patients will be followed during a period of 24 weeks and assesesments as physical examination, questionnaires, 12 lead ecg's, biomarker levels, echocardiogram, Cardiac Magnetic resonance, PET and CPET will be performed to evaluate their response to the intervention.
