Are You a Good Fit for This Trial?
This trial is for adults aged 18 and older with Type 1 or Type 2 diabetes who have recently been released from one of five Connecticut prisons to supervised housing or parole. Participants must be able to speak and understand English.Inclusion Criteria

Screening
Participants are screened for eligibility to participate in the trial
Treatment
Participants engage in a 6-week Diabetes Self-Management Education and Support (DSMES) intervention using the Diabetes LIVE JustICE application
Follow-up
Participants are monitored for self-management behaviors, emotional distress, and metabolic indicators
Open-label extension (optional)
Participants may continue to use the Diabetes LIVE JustICE application for ongoing support
