Participants should live within 10 miles of the study hospital and need ongoing care but not long-term facility placement. Will I have to stop taking my current medications?

The trial protocol does not specify whether you need to stop taking your current medications. However, if you are on methadone or require certain specialty medications or intravenous controlled substances, you may need to discuss this with the trial team.

This trial is for adults over 18 who need skilled nursing facility care after hospitalization. It's for stroke patients not covered by insurance, those recommended for rehabilitative therapy, and surgical trauma patients able to bear weight and transfer with minimal help. Participants must have lived at home before hospitalization and be likely to return home after short-term rehab.

Inclusion Criteria

I can move with minimal help after surgery.
I had a stroke and need rehab not covered by insurance, not qualifying for long-term care.
Lives within 10 miles of any study site hospital (or per specified catchment)

Exclusion Criteria

Clinical: Requires durable medical equipment not already in place at home and excluded below
Clinical: Acute delirium noted by RAH liaison requiring more than one caregiver
My home lacks the space needed for sleep, eating, and therapy.
