CCENT Trial Summary
This trial will evaluate a new model of follow-up care and support for parents of infants who spent time in the Neonatal Intensive Care Unit (NICU). The model, called the Coached, Coordinated, Enhanced Neonatal Transition (CCENT), involves a key worker who provides coaching, care coordination, and education for families during the first year after their child's NICU stay. The goal is to improve the psychosocial support and medical care coordination beyond the NICU, as many of the medical and social concerns do not end at the time of discharge.
