This trial is for adults over 18 with prediabetes who are patients at Johns Hopkins GSS General Internal Medicine or JHCP Internal Medicine clinic and have a primary care physician (PCP) visit during the study period. It's not open to those seeing clinicians other than their PCP.Check my eligibility
