This trial will test the feasibility of a clinical support model to help patients with End-Stage Kidney Disease (ESKD) who wish to choose Peritoneal Dialysis (PD), but who may not be able to do so due to advanced age, physical function/dexterity, vision, cognition, mobility, or psychosocial issues.
