Who is the study for?
This trial is for adults aged 18-85 with high blood pressure who have been treated at a clinic for at least a year and have had an outpatient visit with a hypertension diagnosis in the past 12 months. Pregnant individuals or those with end-stage kidney disease requiring dialysis or transplantation cannot participate.Check my eligibility
What is being tested?
The study is testing whether adding practice facilitation to Clinical Decision Support (CDS) systems helps manage hypertension better than using CDS alone. Forty small primary care practices in New York City will be randomly chosen to use either both methods or just CDS for twelve months.See study design
