Who is the study for?
This trial is for adults with a March, April, May or June birthday who are patients at UCLA Health DOM Primary Care. They must be able to receive SMS messages and have an active MyChart status. Participants should have overdue health maintenance measures targeted by the My Action Plan initiative but can't join if they're under 18, opted out of electronic communication, already scheduled/completed all screenings before receiving texts, or are deceased/hospice patients.Check my eligibility

Eligible, randomized participant will receive an action-oriented, generic SMS-message informing patients that they have a health gap to be closed and prompting them to act soon before they forget.
Group II: Arm 3: Intention-oriented; GeneralExperimental Treatment3 Interventions
Eligible, randomized participant will receive an intention-oriented, generic SMS message informing patients that they have a health gap to be closed and highlighting the importance of doing so.
Group III: Arm 2: GeneralExperimental Treatment2 Interventions
Eligible, randomized participant will receive a brief, generic SMS message informing patients that they have a health gap to be closed.
Group IV: Arm 1: SpecificExperimental Treatment2 Interventions
Eligible, randomized participant will receive a brief, specific SMS message informing patients that they should obtain screenings and tests they are due for.
