This trial studies how support from a patient navigator can help older adults admitted to the Orthopedic Unit with a fracture, to improve care, transitions, satisfaction and more.

Who is the study for?
This trial is for individuals aged 65 or older who have been admitted to the Orthopedic Unit at SJRH for a fracture. It aims to help these older adults with their hospital care and transition back home.Check my eligibility
What is being tested?
The CHARM study is testing if having a patient navigator can improve care and recovery for older adults with fractures. The study compares standard care to those getting extra support from navigators, looking at hospital stay length, post-discharge health use, and satisfaction.See study design
