Eligibility Criteria Checklist
Specific guidelines that determine who can or cannot participate in a clinical trial Must have
older adults age 70+ who attend the primary care clinic, speak and understand English language
Be older than 65 years old

Screening 3 weeks

Follow Up the tool will be used at 4 time points: at baseline recruitment (b1), then 6 months later (b2), then 6 months later following intervention implementation (i1), and 6 months later (i2). scores will be compared pre and post.

This trial will help improve the health of older Canadians by identifying health problems early, providing timely supports, and actively engaging older patients and their caregivers in decisions about their care.
