This trial includes the following eligibility criteria:- You are currently using insulin.You must be 18 years or older to participate.You identify as being of Hispanic/Latino heritage.You have been diagnosed with type 2 diabetes or are at risk of developing type 2 diabetes based on a specific diabetes risk assessment tool.
Research Study Groups:
This trial has the following groups:- Group 1: Hispanic/Latino adults with or at risk of T2D- Control Group
- Group 2: Hispanic/Latino adults with or at risk of T2D- Active Group

This trial has 1 awards, including:- No Placebo-Only Group - All patients enrolled in this study will receive some form of active treatment.
