This trial is for older adults living in certain areas with type 2 diabetes, an A1C level over 7.5%, and who have seen a clinic in the past year. This trial includes the following eligibility criteria:- I am unable to give my own consent for the study.I have a history of type 2 diabetes.You have participated in the CRxCaregiver, CRxHunger, or My Diabetes GOAL trials before.You live in your own home or in a community setting.Your A1C level is higher than 7.5%.
