- You have a recorded diagnosis of type 2 diabetes.Your most recent hemoglobin A1c level is 8% or higher.You have been diagnosed with type 2 diabetes for at least six months.You are between 18 and 70 years old.You identify yourself as being of Hispanic or Latino ethnicity.You are between 18 and 70 years old.You have been to the primary care site at least once in the least twelve months.You have been diagnosed with Type 2 Diabetes and it is in your medical records.
- Group 1: Intervention
- Group 2: Control
- No Placebo-Only Group - All patients enrolled in this study will receive some form of active treatment.
- Screening: It may take up to 3 Weeks to process to see if you qualify in this trial.
- Treatment: The duration you will receive the treatment varies.
- Follow Ups: You may be asked to continue sharing information regarding the trial for 6 Months after you stop receiving the treatment.
