Who is the study for?
This trial is for people over 65 with dementia who are part of the NewYork Quality Care organization, live at home, and had disjointed outpatient care in the past year. It's not for those in long-term care or nursing homes, or enrolled in home hospice.Check my eligibility
What is being tested?
The study is testing two ways to decide who gets help from a care coordinator: one based on perceived need and another based on usual practices like hospital discharge. The goal is to see which method works better for dementia patients.See study design

Study Objectives

Acceptability, as measured by the proportion of participants who are engaged with care
Appropriateness, as measured by the proportion of problems with communication that are identified by participants and that are in scope for the work of care coordinators

Group I: InterventionExperimental Treatment1 Intervention
The intervention group will assign care coordinators to PLWD based on perceived need for assistance with care coordination. Perceived need will be measured through a proxy's responses to a previously validated telephone survey on perceptions of care coordination.
Group II: ControlActive Control1 Intervention
Usual care assigns patients to care coordinators in response to a discharge from a hospital or a direct referral from a physician.

This trial includes the following eligibility criteria:- I am over 65, on Medicare, and part of the NewYork Quality Care group.I've had inconsistent outpatient care in the last year.

This trial has 1 awards, including:- No Placebo-Only Group - All patients enrolled in this study will receive some form of active treatment.
