Treatment Groups

This trial involves 2 different treatments. System Navigator is the primary treatment being studied. Participants will all receive the same treatment. There is no placebo group. The treatments being tested are not being studied for commercial purposes.

Main TreatmentA portion of participants receive this new treatment to see if it outperforms the control.

Measurement Requirements

This trial is evaluating whether System Navigator will improve 1 primary outcome and 3 secondary outcomes in patients with COVID-19. Measurement will happen over the course of 3 months.

COVID related outcomes
Patient reported COVID related clinic or hospitalization visits captured at baseline and 3 month follow up
Social needs met
Number of social needs experienced by patients from a list (yes/no) at baseline and 3 month follow up
Emotional Distress
Measure of emotional distress as measured by anxiety and depression systems.
Mortality and hospitalizations related to COVID-19 and all cause mortality
Mortality and hospitalization data obtained from administrative data source

How many people get mental health a year in the United States?

In 2018, nearly two-thirds (61%) of adult Americans had no major depressive or anxiety disorder, but many did have other mental health symptoms such as problems with stress, trouble sleeping, having trouble concentrating, or having trouble with anger or irritability. Most people had no serious problems with mental health that required them to seek psychiatric help. The number of adults with any mental health problem was highest in those ages 18–29 years. Most Americans had little difficulty with mental health problems due to the lack of treatment.

What is mental health?

It is difficult to define what constitutes mental health. However, some of the common mental disorders that can form a part of this category are depression, substance abuse and anxiety disorders. Mental health is not just a mood state, but may also include social, physical and occupational well-being. In the UK, in 2005, mental health was defined as, 'A good state of subjective well-being with respect to thinking, feeling, behaviour and relationships as a result of the mental functioning experienced by a group or individual' (Office of Population Censuses and Surveys). Mental health, is of importance because it is linked to the social and health outcomes of individuals and society at large.

Common treatments for mental health conditions commonly include behavioral therapies and medication. Those receiving antidepressant therapy commonly experience side effects. People with co-occurring anxiety, substance abuse, or cognitive impairment are more likely to experience side effects from antidepressants. While psychiatric disorders frequently co-occur with physical disorders, no psychiatric disorder is unique to any particular type of disorder.

How does system navigator work?

The system navigator model was able to support and guide both family and non-family members in the clinical process. The use of system navigators in this context of care appears to have an array of benefits. These include not only improved person-centered engagement with treatment, but also improved treatment integrity, greater understanding of illness-related challenges and more effective coordination of services and resources. system navigators may increase the potential for improvement and reduce the potential for harm and error related to treatment. The system navigator is a key tool for supporting care, managing transitions and mitigating treatment conflicts and potentially may increase the likelihood of improved outcomes for clients.

Have there been other clinical trials involving system navigator?

Despite our study being underpowered to adequately refute the hypothesis, we do not think that there is a clear negative association between the presence of SN and the likelihood of receiving or not receiving medication in clinical practice. The lack of statistically significant associations between the SN and medication in clinical practice in our study is perhaps unsurprising as the treatment of mental illness in the NHS is very poorly controlled.

Findings from a recent study shows that the system navigator role has been successfully implemented in the clinic. Given that it is inexpensive, time-efficient and easy to learn, the system navigator may be a useful component in clinical practice for informing the treatment trajectory and optimizing outcomes.

Age of onset of an MHS can occur at any age of development with gender and socioeconomic factors. Women are more likely to present under 65 in China and India and as in other countries. The findings suggest the importance of early recognition and early assessment of MHS among the general population by health care professionals to ensure a well-timed start of a comprehensive therapy for prevention and treatment of MHS and its complications.
