Study Summary
This trial will provide the Complete Health Improvement Program (CHIP) to 25 employees of Ohio University who have diabetes, prediabetes, obesity, hypertension, atherosclerotic cardiovascular disease, or dyslipidemia. The goal is to improve self-management and reduce the consequences of these conditions through lifestyle changes.
