This trial will test whether DynamiCare's program, which combines coaching with financial incentives, is more effective than usual care in reducing alcohol use among adult Medicaid recipients.

Who is the study for?
This trial is for adults over 18 with Alcohol Use Disorder (AUD) who speak English and have a smartphone. They must meet the DSM-5 criteria for AUD, be recommended for care based on ASAM CO-Triage assessment, and have had alcohol within the last 10 days. Those with suicidal intent or needing inpatient treatment beyond Level 3.1 are excluded.Check my eligibility
What is being tested?
The trial tests DynamiCare's digital coaching program against routine care to see if it helps people with AUD. The program uses a smart debit card, remote coaching, and integrates Contingency Management and CBT techniques through an app.See study design
What are the potential side effects?
Since this intervention involves behavioral therapy via an app rather than medication, traditional side effects like those seen with drugs aren't expected. However, participants may experience stress or discomfort when discussing their substance use.

Group I: Untreated or Routine Care Control GroupExperimental Treatment1 Intervention
Control participants will receive substance tests at the same frequency as Intervention participants, and the same incentive amounts for tests as treatment participants. Controls' payments, however, will not be contingent on positive/negative results, but rather only on valid, on-time submission. Controls will not receive coaching, CBT or rewards for appointments.
Group II: DynamiCare Motivation Support ProgramExperimental Treatment1 Intervention
Intervention Group members will receive 32 weeks of remote Contingency Management (CM; financial motivational incentives), Recovery Coaching, substance testing, appointment reminding/tracking, and in-app Cognitive Behavioral Therapy (CBT). After 32 weeks, coaching, testing, appointment tracking and CBT will continue until an overall 12 months in the project is completed.
