AFTER AKI Trial Summary

This trial is testing if structured post-discharge care can improve outcomes for people who have developed acute kidney injury while in the hospital.

Who is the study for?
This trial is for adults over 18 who survived a hospital stay with acute kidney injury and don't have arranged follow-up care with a kidney specialist. They must have had their blood creatinine levels double during the hospital stay, not be on chronic dialysis from before, expect to live more than 6 months, and have a primary care physician.Check my eligibility

Group I: Risk guided follow-upExperimental Treatment1 Intervention
Participant will be stratified for risk of CKD in three groups: Low (<1% risk of CKD), medium (1-10 % risk of CKD) and high (≥10 % risk of CKD). Specific follow-up will be guided by risk status
Group II: Control Arm (usual care)Experimental Treatment1 Intervention
Participants will be discharged as per usual ward discharge protocols.
