Who is the study for?
This trial is for older adults, aged 65 and above, living in Leslie County with Type 2 Diabetes Mellitus (T2DM). Participants must have visited a clinic within the past year and be able to give consent. They should also be in regular weekly contact with someone who can provide social support.Check my eligibility
What is being tested?
The study is testing a 6-week educational program called the Diabetes Empowerment Education Program. It's delivered by community health workers to see if it helps improve self-care and clinical outcomes for rural-dwelling older adults with T2DM.See study design

Rural-dwelling older adults will be interviewed to map their social network structure, determine the types of social support provided by members of their social network, and identify key players within these networks. This group will be paired with their key players (identified during interviews) to receive the diabetes education. The pair complete the intervention as a dyad.
