SPA-LTC Program for Frailty Syndrome
Trial Summary
What is the purpose of this trial?
This study will evaluate the implementation of the Strengthening a Palliative Approach in Long-Term Care (SPA-LTC) program using a cross-jurisdictional, effectiveness-implementation type II hybrid design (dual testing of clinical and implementation interventions) to assess the implementation (feasibility, fidelity, reach, sustainability) and effectiveness (family satisfaction, staff knowledge and confidence implementing a palliative approach to care, hospital use).

Eligibility Criteria
This trial is for English-speaking residents in long-term care who are quite frail, with a score of 50% or less on the Palliative Performance Scale. It's not suitable for those who aren't fluent in English or are less frail (score above 50%).Inclusion Criteria

Participants are screened for eligibility to participate in the trial
Intervention
Implementation of the SPA-LTC program in long-term care homes across three provinces
Follow-up
Participants are monitored for family satisfaction, staff knowledge, and hospital use after the intervention
Post-Bereavement Follow-Up
Follow-up with families and staff after bereavement
