This trial is for cancer survivors aged 21-64 who have had breast, prostate, colorectal, lung or other cancers at any stage from 1 to 4. Participants must have completed some form of cancer treatment in the past year, own a smartphone or tablet and allow access to their digital health records from the last year.Inclusion Criteria
