What You Need to Know Before You Apply

What is the purpose of this trial?

The purpose of this study is to evaluate the clinical effectiveness of Virtually Assisted Home Rehabilitation After Acute STroke (VAST) + post-acute stroke usual care (PASUC) compared to PASUC alone and examine how social determinants of health (SDOH) influence access to and utilization of PASUC rehabilitation.

Are You a Good Fit for This Trial?

This trial is for adults who recently had a stroke (either ischemic or hemorrhagic), are stable enough to go home, need rehabilitation, can speak English or Spanish, and have access to the internet and a video device. They must be able to give consent or follow simple commands if someone else consents for them.

Inclusion Criteria

o those being referred from the community settings must be within 60 days of post-acute discharge.
* Clinically stable and appropriate for discharge per acute care facility
* Planned discharge to home (with or without Home Health (HH)/Outpatient Rehabilitation (OPR), Inpatient Rehabilitation Facility (IRF), or Skilled Nursing Facility (SNF)

Timeline for a Trial Participant

Screening

Participants are screened for eligibility to participate in the trial

2-4 weeks

Treatment

Participants receive Virtually Assisted Home Rehabilitation After Acute Stroke (VAST) + post-acute stroke usual care (PASUC) or PASUC alone

6 weeks
Virtual visits for rehabilitation sessions

Follow-up

Participants are monitored for changes in functional abilities and quality of life

12 weeks
Assessments at 6 weeks and 18 weeks

Trial Overview

The study compares two groups: one gets virtually assisted home rehab plus usual post-stroke care (VAST + PASUC), while the other receives only usual post-stroke care (PASUC). The goal is to see if virtual rehab helps recovery in underserved communities.
