This trial is for older adults with multiple chronic conditions who are leaving Intermediate Care Units. They should be able to complete surveys and participate in follow-up calls. Inclusion Criteria

I am 50 years old or older.
Living with two or more chronic health conditions as identified by diagnosis in the electronic health record
I have a working phone or way to be contacted.
