This trial is for women in the Mississippi Delta, especially Black women, who are pregnant or postpartum. It aims to address disparities in maternal health and healthcare outcomes by building trust and engagement with community-focused interventions.

Inclusion Criteria

Identified as high risk pregnancy
I am a new parent, aged 18-45.
Resident of one of the 5 target counties (Washington, Bolivar, Scott, Humphreys, and Carroll)

Exclusion Criteria

Not identified as high risk pregnancy
Non-resident of the 5 target counties
I am at least 18 years old and have recently given birth.

Timeline for a Trial Participant

Screening

Participants are screened for eligibility to participate in the trial

Intervention

Implementation of a community health worker home visiting program and a multilevel communication strategy to increase trust and engagement in postpartum healthcare

Participants are monitored for severe maternal morbidity rates and ED visits up to 1 year postpartum
