Are You a Good Fit for This Trial?

This trial is for older Veterans who've seen a VA primary care doctor in the past year, take at least 5 medications daily, and have at least 2 chronic conditions. It's not open to those unable to consent or with a dementia diagnosis.

Inclusion Criteria

Had at least 1 VA primary care visit within past year
I take 5 or more medications every day.
At least 2 chronic conditions

Exclusion Criteria

I have been diagnosed with dementia.
I am unable to understand or make decisions about my treatment.
