What You Need to Know Before You Apply

What is the purpose of this trial?

The purpose of this study is to evaluate the feasibility and preliminary effectiveness of a primary care-based Interdisciplinary Addiction Care Transition (IntACT) team that will meet patients with substance use disorders (SUD) during a medical hospitalization and provide intensive care management, peer support, and interim SUD and medical care after discharge while facilitating a transition to long term community-based treatment.

Are You a Good Fit for This Trial?

Inclusion Criteria

Hospitalized at University of Utah Medical Center (UUMC) for any medical reason
Electronic Health Record (EHR) documented diagnosis of any Substance Use Disorder (SUD)
I am 18 years old or older.

Exclusion Criteria

Currently incarcerated
Tobacco use as the only documented substance use disorder

Timeline for a Trial Participant

Screening

Participants are screened for eligibility to participate in the trial

2-4 weeks

Intervention

Participants receive the IntACT intervention or Usual Care for 4 months, including in-hospital discharge planning, post-discharge outreach, and care coordination.

4 months
Weekly outreach and follow-up visits every 1-2 weeks

Follow-up

Participants are monitored for retention in treatment and other outcomes after the intervention period.

2 months
Close-out assessment at 6 months post-randomization
