This trial is for individuals with uncontrolled type 2 diabetes who also have unmet health-related social needs. Participants should be patients at Boston Medical Center where the THRIVE infrastructure exists. The study excludes details on specific inclusion and exclusion criteria.

Inclusion Criteria

Health-Related Social Needs: Must have been screened for health related social needs (HRSNs) during a General Internal Medicine (GIM) visit in the last 3 months and screened positive for at least one HRSN
I have been diagnosed with Type 2 Diabetes Mellitus.
My HbA1c level is 9% or higher, indicating uncontrolled diabetes.

Exclusion Criteria

Patients enrolled in Complex Care Management (CCM)
I am currently receiving hospice care.
This criterion is not applicable to living individuals.

Timeline for a Trial Participant

Screening

Participants are screened for eligibility to participate in the trial
