Inclusion Criteria

Had a recent HbA1c of 7.0% or higher
I am between 16 and 19 years old.
I have had type 1 diabetes for over a year.

Exclusion Criteria

Have had a pilot program AHEAD clinic visit
Most recent Usual Care diabetes visit was with a current AHEAD provider

Timeline for a Trial Participant

Screening

Participants are screened for eligibility to participate in the trial

2-4 weeks

Treatment

Participants receive either the AHEAD Program or Usual Care, with clinic visits every three months

15-24 months
6 clinic visits

Follow-up

Participants are monitored for changes in glycemia and participant-reported outcomes after the intervention period
