This trial is for older Veterans who have functional impairments and are transitioning from hospital to home. It's designed to help them regain and maintain independence after hospitalization.

Inclusion Criteria

I am 65 years old or older.
Able to provide informed consent
My physical ability score is between 4 and 9.

Exclusion Criteria

Plan to discharge anywhere except home (e.g., assisted living, group home, or skilled nursing facility)
Admitted to VA Boston for acute psychiatric care or substance abuse detoxification

CTraC-FIT Intervention
