Study Summary

This trial will test whether a new patient portal can help patients with diabetes get needed care. The portal will notify patients of when they need care and allow them to order it.

Who is the study for?
Adults aged 18-75 with Type 1 or Type 2 diabetes, who can read English and use a mobile device with internet. They must be patients of participating clinics with an active My Health at Vanderbilt account. Those on dialysis, with severe vision problems, pregnant or planning pregnancy, or unable to use mobile devices cannot join.Check my eligibility
What is being tested?
The trial is testing a patient portal intervention designed to notify diabetic patients about due preventative care tasks and allow them to order these services themselves. It aims to reduce the number of patients missing important diabetes-related health checks.See study design

Since this study involves using a digital platform for communication rather than medication or medical procedures, traditional side effects are not expected. However, there may be indirect effects related to stress or anxiety from managing one's own care reminders.
