Study Summary

This trial will compare two different types of Shared Medical Appointments (SMAs) for patients with diabetes - one that is patient-driven and one that is standardized. The curriculum for both types of SMAs will be the same, and will consist of six sessions focused on self-management of chronic illness.

Group I: Patient-driven SMAActive Control1 Intervention
In the patient-driven SMA model, patients receive the same TTIM curriculum, but patients at each practice are able to set the order of the curriculum and dictate how long to spend on each topic.
Group II: Standardized SMAActive Control1 Intervention
The standardized SMA model includes the same TTIM curriculum as in the patient-driven model, but it is delivered in a standardized way (order of and time spent on topics are set) across all participating practices.
