S4C Trial Summary
This trial will test the effectiveness of a digital storytelling intervention in promoting self-management of type 2 diabetes among Somali adults. The intervention is based on community-based participatory research and narrative theory, and was created through a partnership between Somali communities and academic institutions.

- I have been diagnosed with type 2 diabetes for at least 6 months.Your most recent hemoglobin A1c level is 8% or higher.My medical records show a diagnosis of type 2 diabetes.I am between 18 and 70 years old.I have seen my primary care doctor in the last year.
- Group 1: Control
- Group 2: Intervention
- No Placebo-Only Group - All patients enrolled in this study will receive some form of active treatment.
- Screening: It may take up to 3 Weeks to process to see if you qualify in this trial.
- Treatment: The duration you will receive the treatment varies.
- Follow Ups: You may be asked to continue sharing information regarding the trial for 6 Months after you stop receiving the treatment.
