- You are able to walk.You have a serious kidney or liver disease.You have severe heart failure.Your body mass index (BMI) is higher than 28 kg/m2 and you have at least one additional risk factor for cardiovascular disease (such as high blood pressure, high cholesterol, or smoking) as determined by a tool called LS7.You have the ability and willingness to use digital technology.
- Group 1: Usual Care then AF Care
- Group 2: Pilot Testing
- Group 3: AF CARE plus Usual Care
- No Placebo-Only Group - All patients enrolled in this study will receive some form of active treatment.
- Screening: It may take up to 3 Weeks to process to see if you qualify in this trial.
- Treatment: The duration you will receive the treatment varies.
- Follow Ups: You may be asked to continue sharing information regarding the trial for 6 Months after you stop receiving the treatment.
