This trial is testing whether case management can help women who have had a heart attack, heart surgery, or other cardiac problems to participate in and stick with a cardiac rehabilitation program.

Who is the study for?
This trial is for women living in the Burlington, Vermont area who have had a recent heart event or surgery and plan to stay local for the next year. They should not have attended cardiac rehab recently and must not suffer from severe non-cardiac diseases, life-threatening heart rhythms, advanced cancer, dementia, or frailty.Check my eligibility
What is being tested?
The study tests if early case management (CM) helps women participate more in cardiac rehabilitation (CR). CM includes home visits and counseling on lifestyle changes like physical activity. The goal is to see if this support improves CR attendance and health outcomes after a cardiac event.See study design

Since the intervention involves case management rather than medication, typical drug side effects are not expected. However, increased physical activity as part of lifestyle changes may cause muscle soreness or fatigue initially.
