- I have been diagnosed with severe dementia.My severe heart rhythm problem is under control.I have a heart condition such as a recent heart attack, surgery on my heart, stable chest pain, or weak heart pumping.You have advanced cancer, are very weak, or have a serious illness that may limit your lifespan.I have a condition like severe arthritis or paralysis from a stroke that limits my ability to exercise.
- Group 1: Case management
- Group 2: Usual Care
- No Placebo-Only Group - All patients enrolled in this study will receive some form of active treatment.
- Screening: It may take up to 3 Weeks to process to see if you qualify in this trial.
- Treatment: The duration you will receive the treatment varies.
- Follow Ups: You may be asked to continue sharing information regarding the trial for 6 Months after you stop receiving the treatment.
